Plans & Pricing

We offer standard pricing, wellness packages, and we are in-network with select insurance providers. Explore our pricing and packages below to find an option that works best for you.

Standard Pricing

New Patient Appointments = $200

Includes the initial exam fee ($135) and regular visit fee ($65).

New appointments are 30 to 45 minutes. â€‹

Regular Visits = $65

The regular in-office visit is $65.

Wellness Packages

Wellness Membership Care Plan

Welcome to our Wellness Membership Care Plans! These packages are designed to provide you with exceptional support on your health journey while offering affordable pricing to help you optimize your health and well-being.​

Package One

One (1) visit per month @ $60

A total of 12 visits at @ $60 each = $720 ($60 savings from our standard pricing)

Payments must be prepaid. Includes the option to be billed in three installments of $240.

Package Two

Two (2) visits per month @ $55 

A total of 24 Visits @ $55 a visit = $1320 ($240 savings from our standard pricing)
Payment must be prepaid. Includes the option to be billed in four installments of $330.

Insurance Providers

We are in-network with the following insurance providers: Medicare, Kaiser PPO, First Choice, All Savers, and United Healthcare Inc. To confirm if we accept your specific plan please call your provider to find out what's covered on your plan prior to your first visit. an estimate of your coverage. As a service to you, we will file your insurance claims on your behalf. Your insurance policy is a contract between you and your insurance company. Benefits quoted to us are not a guarantee of payment. 

Patient Responsiblity

You are responsible for:

  • Any co-pays, deductibles, or coinsurance required by your plan.

  • Any services not covered or denied by your insurance. 

  • Any remaining balance after insurance has processed your claim​ 

Non Payment by Insurance

If your insurance company denies payment or fails to pay within a reasonable time, you agree to be personally and fully responsible for all charges incurred. 

Changes in Coverage

​It is your responsibility to inform Align Chiropractic of any changes in your insurance coverage. 

Collections Policy

If your account is unpaid after reasonable attempts to collect, your account may be sent to collections, and you will be responsible for any additional collection or legal fees. 

Our Office Policies

Payment Information

We strive to make the financial aspect of your care transparent and manageable.

Payment is due at the time of service.

 

We accept the following forms of payment: Cash, Major Credit Cards (Visa, MasterCard, American Express,

and Discover), Health Savings Account (HSA), and Flexible Spending Account Cards (FSA).

We are in network with select insurance providers and also offer wellness packages to help offset the costs of your health care. 

Cancellation and rescheduling

We understand that life happens and schedules can change. If you need to cancel or reschedule your appointment, please provide us with at least 24 hours' notice. This allows us to offer the appointment slot to another patient who needs care.

Appointments canceled or missed without providing 24 hours' notice may be subject to a cancellation fee. We appreciate your cooperation in helping us serve all of our patients effectively.

Patient Confidentiality (HIPAA) 

Your privacy is of the utmost importance to us. Our practice is fully compliant with the Health Insurance Portability and Accountability Act (HIPAA). All your personal and medical information will be kept strictly confidential and will only be released with your express written consent. You will be provided with a copy of our full HIPAA Notice of Privacy Practices during your first visit.
